ICON is a contract research organization: pharmaceutical, biotech and medical device companies hire it to run their clinical trials instead of building the capability in-house. ICON recruits patients, manages trial sites in dozens of countries, collects and analyzes the data, and prepares the regulatory submissions that get a drug approved. It does not develop or own any drugs itself — its business is running other companies' research, under contracts that can span years and multiple trial phases.

Contracts are typically fixed-price or unit-based, spanning the multi-year life of a clinical trial, and revenue is recognized as work is performed rather than at signing. A large share of new business comes from ICON's existing large pharmaceutical clients rather than new logos, so revenue depends heavily on how much those clients keep spending on drug development and how many of their trials get cancelled partway through — a 2025 accounting investigation found the company had been recognizing some of that revenue too early.
